#2 is "being with a partner. Locale is the South-West in the Mid-summer & it's very hot! You & your best friend are experienced rafters & are going on a journey down on of the rivers that flow through the scenic countryside. Then you hit a bad patch of rapids & everything goes South! The raft turns over, spilling you, your friend & your gear into the water. You & your buddy
wash up on a bank miles downstream. The good news is that you had survival equipment in 2 dry-bags lashed to your bodies. (1 each). They & what you have on your belts & in your pockets are the only gear that made it. The bad news is that your friend is screaming in pain from the compound fracture of his right lower leg. You can see bone sticking out of the wound! Also there are symptoms suggesting some broken ribs, and his eyes show uneven dialation, suggesting a possible concussion. Worse, while scouting the immeadiate area you discover what look to be fresh Cougar tracks. What do you do?:cool2:
Easy......I would not have gone on this trip without check points and scheduled info. given to the local authorities and family.
Stop the bleeding, treat for shock, get on high tech radio that would have been on my belt sealed against the elements. Start a signal fire, boil some water, get a major amount of signaling fire material ready.
Once that is done, make a sort of LZ if possible or clearing for the emergency evac.
And hope for the best.

Treat the friends wounds as best as possible. Without knowing the exact area I don't know for sue whether I would wait for others to come along or go to get help.

Stop the bleeding, treat for shock, immobilize the leg, make as comfortable as possible (breating may be labored), cool compresses for the head. I am not leaving. We would have given somebody our route and exit times, so I know that help will eventually be on the way. If within cell phone coverage, call for help (it was in my dry bag). Start a fire to begin signaling - make lots of smoke. Look for additional signs of concussion (memory loss, nausea, lack of swelling to the area of the head that suffered the trauma). Continue making preparations for a stay - shelter - more fire material. Continue to assess and evalutate condition of partner. Stay calm and keep partner calm. Get some clean drinking water.

Your partner is sorta screwed. It depends on how long away I am from a telephone/ civilization. If within an hour or two walk, first aid for us, fashion a spear, make a signal fire near my buddy and walk out to get help. If more then a few hours then I would sit tight.
That's a bad scenario for the partner. Like everyone else, treat the injuries, make comfortable, call for help and wait it out.

The pain should subside significantly once traction is applied, and pain relief alone is a significant factor in minimizing shock.
The ribs - well, is there any sign of pulmonary/internal bleeding?
The concussion? Keep your friend immobilized and hydrated, and apply a cold pack, if you have one in your kit, to the head. Tylenol is also indicated, if in your kit.
For the shock, body temperature must be maintained and any bleeding controlled. Adaquate hydration also, but don't overdo it (surgery is likely). Maybe setting up some shade from any direct sun is in order?

You don't want to apply traction to an open fracture.
When a femur is fractured, the strong muscles of the thigh contract causing the broken bone ends to slide over one another. Without proper treatment and splinting the strong contractions of the thigh muscles may cause the jagged bone ends to further lacerate the tissues and possibly turn a closed fracture into an open fracture. Traction splints provide realignment and stability to the bone ends, reducing bleeding, swelling, and pain. Therefore, do not hesitate to apply a traction splint to an open femur fracture. Because there are several different types of traction splints the EMT must become familiar with the type used by their department. Page III–177 describes care of fractured femurs in more detail.
The following general applications guidelines apply:
Use the traction splint when there is not involvement of the hip, pelvis, knee or lower leg. Remember while applying a traction splint you must never become so completely engrossed in the process that you fail to notice changes in the patient’s ABCs.
Remove or cut away clothing and cover any wounds with a sterile dressing. Manually immobilize the injured extremity while doing the preceding. Check distal pulse, movement and sensation before any manipulation.
Measure the splint against the uninjured extremity, extending the splint only the distance necessary for proper splint function. Remember helicopters and some other transport vehicles have very limited space to accommodate the splint.
Position the splint against the injured extremity. Open and position the ischial pad and straps on the splint; ending up with a strap above the fracture site, one below the fracture site, one just below the knee and one just below the calf muscle. Apply the ankle hitch to the injured extremity while providing manual immobilization avoiding any undue movement to the injured extremity.
Prepare to apply traction by first immobilizing the upper thigh/femur in a counter-traction maneuver. Quickly realign/ reduce the fracture and simultaneously raise the thigh just high enough to get the splint under the leg during the process of applying traction. This process will cause pain, but there will be significant decrease in pain once the fracture has been reduced. The ideal location to apply traction is just above or just below the knee (this location reduces pivoting movement at the fracture site, promoting better stability of the fracture site). If the EMT is unable to maintain a secure grasp above or just below the knee for the purpose of applying traction then the EMT may move down the leg to a point at which they can maintain a secure grasp.
Position the splint in place by sliding it under the extremity with the ischial pad against the patient’s ischial tuberosity (where the buttock and thigh meet). The groin strap is secured first; padding it before securing it. Then attach the ankle hitch to the splint. Turn the winch allowing the splint to completely take over traction before releasing your manual traction. Never release manual traction once you have applied it until the splint is properly applied.
Secure the velco straps in their proper locations. Recheck distal pulse, movement and sensation. Elevate the injured leg by raising the splint stand. Repeat vital signs.

Uh, Ken...reread Sarge's post. The wound IS an open fracture...We can see the bones sticking out. Otherwise excellent info!!
This advice IS for a open fracture. Note the BOLD PRINT in my post above.
Your immediate concern in this scenario is shock, which can kill. Pain severely exacerbates shock. The risk of infection which is very real, won't make much of a difference if the patient first dies from shock at the scene. A hospital can treat for infection later. Morgues don't offer any valuable treatment at all.
Would my advice be different if an ambulance were on the way. Yep, it would. My advice was base on my training and my years of experience in the field. I stand by my advice.

Yep. You're correct. My error. No traction for a tibia fracture. Time for new reading glasses. :innocent:
However, I would still apply traction if it were the femur.

People don't just break the femur, they break the femur, have a brain bleed, cardiac contusion, all conditions that require rapid transport to a hospital. Staying around to set up a traction splint may not be the appropriate choice.

Under the scenario, rapid transport seems unlikely. I've treated several broken femurs which involved "only" broken femurs and minor associated injuries. There's no way I'd leave this guy until he's at least stabilized.

That's why we first try to SIGNAL for help while we tend to the patient. My take is we aren't even sure exactly where we are, except that we're miles away from help. I'd be more concerned about controlling the bleeding and shock, and even the cougar, than I would be about running off to try to get help.
Years ago, the ambulance service was "load and go." Today, with EMS, we treat at the scene and stabilize to the extent possible before transport.

He does expect us to meet the golden hour, he prefers a half hour.

yes it was. I could treat him in five to ten minutes, start a fire in 2 get extra wood for him to use in 3. fashion a spear while walking.

Alot of what I'd do depends on my knowledge of that particular river corridor, and the current temperature, forecast, and time of day. First thing though would be to stabalize him to make sure he isn't in a position or situation to further complicate his predictament. So, I'd make sure he is warm, as dry as possible, and being treated for shock. Than, I'd continue to monitor his vitals and devise a plan.
My plan would depend on a few things....
First, if its one of the well-traveled rivers (by other paddlers) of the SW like the San Juan, Colorado, Salt, Rio Grande, Green, etc... my strategy would be different than if it was a river that receives little floating pressure.
Also, it would depend on if it was a weekend like Memorial Day, or if its just some random Tuesday afternoon. How likely is it that someone else will float by soon? If its likely- I would stay put and take care of my friend (following much of the advice already given) and wait on another paddler to come by and send them out for ALS.
If it was a waterway with little floating pressure, what I'd do would depend on my knowledge of the immediate area. Again treat the injuries as already posted, but then decide- How fast could I hike-out? Could a helicopter land were my pal is? Could I pinpoint the location of my buddy for rescuers? and again temperature and time of day. If it was early in the day, warm with no chance of rain and I knew that a road was close by, I would set-my buddy up as comfortable as possible, clear a place for a helicopter, make a signal recognizable from the air, then take- off running to get to that road. I am not worried about the cougar. Cougar attacks on humans are rare, and cougars generally hunt at night. Also, cougars attacks are often triggered by fleeing prey- they normally don't pounce on something just lying there. The cougar in my experience is not much of a worry.
If the chance of running to a road in reasonable time was not likely, I'd hatch a evacuation plan of my own, and get my buddy out. Most likely the raft is just below the rapids in an eddy, so I'd go get some supplies out of the raft, that I could use for evac. Paddles, sleeping bags and rope can be used to make a servicable litter, so I could drag him out. Maybe there is a railroad track nearby- often times rail lines run parallel to rivers- I could lay the improvised litter sideways across the tracks and drag him out. Or, if its a generally placid river, I could always create an improtu stretcher, stablize him put him in the boat and paddle him out. I'd keep monitoring his vitals, and keep moving trying to keep him comfortable as possible, and adjust my level of care depending on how he's doing and hopefully make it to a bridge or parallel road before he really starts to deteriate.

I threw this scenario at some of the guys at work and they mentioned something that I should have thought of. If I am in this scenario I would want a better assessment then the one given. Being a professional I would be able to do this. Jason montana brings up some good points as well.
I like these little scenarios. we should have more.

Here's my question. If after you stablize the injuried party couldn't you fashion some sort of litter and take your buddy with you or would it be to dangerous to move him? If he could be moved I think this would be better than leaving him by his self. If this seems like a dumb question please forgive me, I'm just asking so I can learn.

Not a dumb question at all. With the injuries listed, taking him/her on a bumpy ride out might further injure or kill them. If it is your only option then maybe, but if you took all of your pre-trip precautions, help will be on the way. You just need to signal so they can find your exact location. Another factor working against dragging them out on a litter (bouncing all the way) is the extreme heat conditions. In addition to the weight of your partner, you will need to carry and consume a lot of water.

Why not make a raft to float you friend out instead of dragging him thru woods?
Would be alot easier on both you

TR - you can "what if" a scenario to death or until it yields a desired outcome. Unfortunately in life we are dealt the hand we must play and don't get to draw cards from the deck. In the situation you describe if materials are available and you know what is down stream that may be the best option.
