    I'll bet he is a left handed shooter and pulling it to the left. I'm right handed and if I shoot a handgun offhand, I sometimes pull off to the right if I haven't shot in a while.

    I've seen a lot of good theories and suggestions but fact is that's all they are until you can narrow it down on your own or have someone who knows what they're doing watch and coach you. The two best I comments I saw based on your description were grip/trigger finger placement and SQUEEZE the trigger. The exact moment the gun fires should surprise you. After you achieve a consistent grouping you see where the gun shoots and determine if it's something you're doing or they all actually shoot to the left. My money goes on something in your grip/trigger finger or jerking the trigger is your problem.
