It is small but at the very least it could be useful for storing insulin & other meds like it.
Evaporative cooling has been used for a long, long time. http://en.wikipedia.org/wiki/Pot-in-pot_refrigerator Clay pots are common vessels. Looks like she built it into a nice, neat little package. Very nicely done.

Same principle as the radiator water bag. I remember using those on a trip across old 66.
